For actively-at-work, benefits-eligible employees who are hired or
become benefits-eligible on or after March 3, 2008.
This voluntary employee benefit is offered to benefits-eligible employees,
spouses/domestic partners (same or opposite gender) and other family members.
During the first 31 days of benefits eligibility, the employee may obtain coverage
without any medical underwriting. All other family members will be subject to some
level of underwriting.
IMPORTANT: After the first 31 days of benefits eligibility, the
employee's acceptance is subject to full medical underwriting. This plan does not
offer an annual guaranteed acceptance open enrollment.
